Online Pre-Surgical Questionnaire

To help us server you better, DMH now has an easy and convenient anesthesia patient questionnaire to complete online before your surgery.

1. Go to DMH's Patient Portal website, https://mydoctor.dmhhs.org
2. Click on Pre-Surgical Questionnaire in the left navigation "Links" section.
3. Enter the account number given to you during your phone registration and enter the month, day and year of your birth.
4. Verify your name and birthday.
5. Follow the prompts to complete your pre-surgical history.
